 Agreement: I understand tuition is billed quarterly with the card I provide here. First qtr: Sept-Nov; 2nd Qtr: Dec- Feb; 3rd Qtr: Mar-May, DUE on the 1ST of each quarter. (If I need to make monthly arrangements, I will make those arrangements). After the 5th, $10 late fee is added per week overdue, per child. Payment options: 1) Debit card preferred, or 2) Credit card entered here, with the frequency selected. Or you may, 3) Venmo @dancersplace, or 4) Checks or cash to The Dancers' Place (after paying the required Registration fee first on this payment page.) I understand we are registering for the whole school year, Sept-May of that year, &/or the summer sessions selected. If I cancel mid year, I agree to pay the next full month of tuition, then we will be released from payments/commitments as TDP tries to fill our spot. Returned Checks $35. Also, I understand a full month tuition amount is due whether there is 3, 4, or 5 classes in a month. If there are only 2, only 1/2 tuition will be due. .............................................................................................................................................................................................................. REGISTRATION FEE: FALL-$60 ($20 is bookkeeping, $40 towards one costume). I understand I am not fully enrolled until I pay my fee and tuition, and there is limited space in each class. Registration/costume dues are non refundable. The only other cost outside of costumes, is a $10 per Family Concert fee, assessed with second quarter tuition, December 1. ...................................................................................................................................................................................... .................................... LIABILITY WAIVER: The undersigned certifies that he or she is the parent and or legal guardian of the above minor child. I give permission for the above name student to participate in the program offered by The Dancers' Place, TDP. I hold harmless the owners, teachers, and instructors/aides of The Dancers' Place from all injuries, harm, or loss of personal property that may arise from any participation at the studio and agree to not pursue any legal action against them. I further authorize any representative of The Dancers' Place to obtain any medical treatment that may be deemed necessary. I know that participating in classes at The Dancers' Place can result in injury and can cause possible harm. I read, understand, and agree to the policies outlined in The Dancers' Place studio guidelines. I further give permission for any photos and videos taken during this program to be used for promotion of The Dancers' Place. ...................................................................................................................................................................................... WHAT TO EXPECT FROM HERE? You will receive a confirmation page that you signed up and paid. (Check your junk mail if you don't receive that.) Once your registration and tuition is paid, your spot is secure and you are officially in your registered class. Also, look online at www.dancersplaceut.com at the "Calendar/Important Info" tab on general information, what to wear, drop off policy, etc. As mentioned above, you aren't fully registered until your form of payment has been received.
